RELAPSE PREVENTION & AFTERCARE
A relapse prevention and aftercare plan is a vital, written tool for anyone in recovery. Having a plan helps you recognize your own personal behaviours that may point to relapse in the future.

Relapse prevention and aftercare plans are written with the help of Liberty Intervention's counsellors. The plan offers a course of action for responding to triggers and cravings. It also outlines ways to combat harmful and self-defeating behaviours that will help clients back on track.

RELAPSE IS NOT USUALLY A SPONTANEOUS EVENT.
Typically it is a four-part process:
Emotional relapse
Mental relapse
Physical relapse
Spiritual relapse
THE MAKING OF A RELAPSE PREVENTION PLAN
Relapse is common regardless of how many months or even years a person may have been in recovery.
Liberty Intervention's trained counsellors are professionally skilled at making relapse prevention manageable and less confusing offering a safe and judgement-free environment supporting the needs of each individual.

CONSIDER THE FOLLOWING WHEN CREATING A RELAPSE PREVENTION AFTER-CARE PLAN:
ASSESSING THE RECOVERING ADDICT'S HISTORY WITH DRUGS AND ALCOHOL AND, MORE IMPORTANTLY, EXPLORING FAMILY HISTORY, TRAUMA AND GENERATIONAL PATTERNS OF ADDICTION.
PERSONAL TRIGGERS KNOWN AND UNKNOWN, ARE A VITAL PART OF ADDICTION RECOVERY. LIBERTY COUNSELLORS BRING AWARENESS TO CLIENTS DEALING WITH TRIGGERS USING A UNIQUE TEACHING METHOD.
CREATING AN ACTION PLAN ESTABLISHES CLARITY AND CONNECTION AND IS WHERE A CLEAR PATH EMERGES. THIS PATH IS PART OF OUR CLIENTS' UNIQUE ACTION PLAN.
